About the 'run' thing... I was thinking maybe the doctors told her to act out that little bit. It could have been based off of Teddy's suspicions that he'd been talking about in order to nudge him further into his fantasy. The doctors could've bribed the lady with a treat or something.

I think he was on the ferry to start the "game" - and to confuse the audience. Otherwise, I'm sure we would've picked up on it sooner if he was on the island from the get go. According to what the doctors said, even the doctor in the cave was made up - she had the same letters in her name as his...wife was it? I already kind of forget o.o I'm not sure about the lady who wrote 'run'. I'm still trying to figure that out.
I'm sure the book will clear some more things up. I'll be sure to tell you about it.

and yeah I get what youre saying, but the thing I kind of don't understand was that if they were all in on it and everything, why was there a scene where he was on the Ferry at all? He didn't imgaine that because his doctor/partner was with him. And why did that other lady write "run" in his book if he was a patient there, and did he imagine the lady who was actually the doctor in the cave?
I dunno I think it can go both ways movie-wise.

I saw Shutter Island the day after it opened. I personally loved it and am now reading the book. So far it's very similar. The entire movie was 'fake'. Teddy has been on the island since he killed his wife. Basically, the staff was playing a game with him (letting him play out this fantasy he had about something evil going on) in hope that he'd realize who he really was and what he did. They said this at the end in the lighthouse. He was really Andrew Laedis (or however you spell it) and he killed his wife - not a fire. The 'escaped patient' who drowned her children was actually his wife, but he created this other character trying to escape from reality.

I had a typo in the title... I meant to write seen, not sen. sorry haha -_-
