I'm with Fae. I very rarely do character pages because I write sci-fi fantasy, so many of the characters are either human with a lot of cool equipment and gear, or nonhuman. If I wanted pictures I'd have to draw every one, and while I love to draw, that's something that'd take a while.
Not totally with you on the second one because sometimes it's hard to find face claims (been there, done that, and 99% of the time, call it quits) and sometimes it makes people uncomfortable (I've met people like that before) so you shouldn't hold that against an author and miss out on a good story because of that, I think. I do understand the first one on a level because the default layouts are not aesthetically pleasing in the least, but not everyone knows how to use the layout maker because it's really not as easy as people think and the premade layouts are something you have to look for so aren't known about to new Mibbians. I deal with it if the story sounds interesting.
Other than the first two, I agree completely. If your story is just like every other story or your characters don't draw people in, I think you end up with a really boring story or an exciting story that readers still can't really connect with.
