Hoping someone can help me

Okay so I've seen on several stories at the bottom of a chapter, it has like the characters name in bold and you can click on it and it takes you to a page whether its what they are wearing or what they look like. I have been looking all through out the forums and tutorials the last couple of hours trying to figure out how to do it and have had no such luck at all. I hope i'm making sense and didn't confuse anyone. I'm going nuts here and want to figure it out lol. I hope someone can help me!! Thanks :)
