Johnny the Homicidal Maniac

Johnny has a voice inside of his head, he doesn't know if it's real or if he is just insane. He meets a new neighbor due to an accident that he caused to his closest friend. Then one day See's a man in an ally the "voice" takes over and Johnny himself stops himself. Is there something that went wrong or is this man he save supposed to help Johnny over come this voice in his head?
