i would hint that you might not be coming back after tuesday... say you have a hospital appointment, and then the ball's in their court if they want to know or not... explain that you mnight be hospitalised.

I wold tell them, if they really are great friends as you say they are, they're not going to assume false things. It's better to be honest with thing, especially if it's something involving your health and what not.
