I'm going to go with he really is cheating on her. Maybe married life just isn't what he wanted it to be. He saw his friends starting to get married and felt that he should as well. There can be a feud from ages ago if you want. We would just have to decide what it was?

That could be good. Maybe they were both chasing after the same guy in college or whatever, which leads to Jo not trusting Mel and vice versa. Also, maybe Mel's keeping Zacky's secret not only to protect him, but to annoy Joanna. She likes knowing something that Joanna doesn't. Or maybe one got expelled from college because of what the other did?
