I'm working on a tutorial on how to make CSS/HTML layouts like the one one my site, but it isn't finished yet.
As far as putting them on a website, do you mean how to make a preview for a layout? or just how to start a site in general?

I got mine from asking randomly on a layout-requesting thread in the profiles section of the forums. There's a poop ton of those Mibba layout websites out there, though.

The only downside is that they send you relentless emails after you give them your email address. I used one in January and at least once a week I'll get two or three emails from them.

There's one across the, um...I think it's somewhere near Mandarin.
But the card is usable on Barns & Noble's website. :3

Hells yeah! First of all, your dogs were freakin' adorable - especially the fluffy one (Izzy, right?). And your cats were so friendly and nice, unlike mine. xD
