I think... if it were my story, which it's not (cuz it would be a lot worse if it were), she'd live. Reason being this. Blake's life sucks. He has no money and a poor family. He loses his job, his girl, and it would be way too much for him to loose his sister. The only thing is, if she lives, things can't just become perfect all at once. If he got home from the hospital and Lizzie was waiting right then to confess her love for him... haha, I'd probably just stop reading the story right then and there. It just seems like maybe this one good thing could happen to Blake to prove to him that some things just make life worth it.
