I think even though now she knows Alex loves her she is still scared of help, mostly because she's scared of nobody being able to help her, and not wanting Alex to know what she's become. Ali feels pathetic in a way I guess, she's scared he won't want to be with her any more, which is kind of ironic because being with him was why she was so insecure in the first place about her body.
