This is beautiful... I know the struggles of depression, and I know how hard it is. This is the perfect representation as to what it's like. Thinking fantasy is reality is a common thing for me. I absolutely love this. The skeleton is perfect, its voice is like ice running down my spine. I literally shivered every time it spoke.
To wake up and find yourself in a mental institution after a series of dreams like these... I've had that experience. It's terrifying at first, because you don't know where you are or why you're there, but then you remember... And all you want to do is die...
