Please, please put more punctuation and actual paragraphs and paragraph breaks in. The lack of punctuation and formatting is making this hard to read. This whole chapter 4 has like 11 periods. Starting a new line of text does not make up for lack of punctuation.
Also, physicians have a Doctor of Medicine (M.D.) degree, not a PhD. a PhD makes one a doctor of philosophy in a particular field but never a medical doctor. Medical doctors with PhDs have to both graduate from medical school, pass the requisite tests, and go through a PhD program.
